  • Call 970-704-2760 or 970-328-8840 to request an application

  • Pick up an application at any Eagle County or Pitkin County Health & Human Services office

  • Check if you are eligible online and apply using Colorado PEAK's website

      (You may still need to turn in your completed application to the HHS office.)

 

  • There is a PEAK kiosk available at both Pitkin and Eagle County HHS Buildings to apply online.
