The Senior Care Management Program assists seniors and their families in navigating the various systems and services for medical, financial, physical and emotional care.
These services are available to Pitkin County seniors and on a limited basis for local residents caring for seniors living outside of this community. Services are offered free of charge.
Our goal is to help create and maintain a safe and healthy environment that supports the highest possible level of independence for the senior adult and to enrich the lives of those experiencing life changes.
